How to apply for medical treatment in the U.S.
If you are seeking medical treatment in the U.S., you must qualify for a (B1/B2) nonimmigrant visa and also provide:
Evidence from your physician regarding the diagnosis of your condition and the required treatment.
Evidence that you have been accepted for treatment at a licensed U.S. medical facility for which you must have a scheduled appointment.
An estimate by the medical institution of the cost of your treatment and convalescence, including outpatient care, if you would not be able to return to India immediately.
Evidence that you have sufficient funds to finance your medical treatment and cost of living expenses, during the period of your treatment and convalescence.
If the funds for your medical treatment will be provided by a relative in the U.S., Form I-134 (Affidavit of Support) must be provided along with evidence of the income and assets claimed by your sponsor.
